Communicable Disease Reporting and Prevention
Here at Pickaway County Public Health we strive to protect our community from a number of communicable diseases as outlined by the CDC and the Ohio Administrative Code. We work with local doctors, nurses, administrators, healthcare facilities, and community leaders to protect our county in a way that is fast, efficient, and effective.
A list of reportable communicable diseases, case report forms, and the time frames for reporting are listed in the Ohio Department of Health’s Infectious Disease Control Manual. The first three sections provide valuable reference materials on Ohio’s reportable diseases.

A number of forms are available for the reporting of specific types of illnesses, but the generic Infectious Disease Tracking Form is provided here for ease of use.
We need your help! County residents play a major role in identifying and reporting communicable diseases at the local level. It is imperative that members of local communities are empowered with the knowledge and resources needed for fast and effective transfer of information. With a wealth of informational resources at our disposal, we will do our best to meet the educational needs of any county resident, medical professional or otherwise.
