PLEASE READ BEFORE FILING YOUR COMPLAINT

Pickaway County Public Health takes all complaints seriously and will investigate each complaint that we are able to according to the Ohio Administrative Code and the Ohio Revised Code. Please keep in mind when filing a complaint, you need to fill in your contact information. We cannot take anonymous complaints through our office. If a complaint is filed anonymous other than for a food facility, we are unable to investigate.
When filling in the complaint form, please keep in mind if there is no box to check for the complaint you want to report, the Health Department does not investigate that type of complaint. Ex: Mold, plumbing, roofs, electrical, etc.

For all nuisance complaints, we must have the contact information of the person reporting. If this complaint is related to a licensed establishment (i.e. Food Service, Swimming Pools, Campgrounds) it can be anonymous (please put N/A for each item). Any complaints regarding a sewage system or well must have a reporting name and number. Also, by submitting this complaint, you may be required to testify in court if the complaint is valid and cannot be abated by other means.
