Community Health Assessment (CHA)
The Community Health Assessment is how we take a close look at the health of Pickaway County. We review local data, gather community feedback, and talk with partners like schools, hospitals, and community organizations to understand our biggest health challenges and opportunities.
Community Health Improvement Plan (CHIP)
Using that information, we create our Community Health Improvement Plan. This plan outlines clear goals and action steps to improve priority issues such as mental health, substance use prevention, chronic disease prevention, and overall wellness. We work alongside community partners to turn these goals into real results.
Our Strategic Plan
While the CHA and CHIP focus on community health priorities, our Strategic Plan ensures we are organized, accountable, and prepared to serve you well. It guides how we strengthen our workforce, manage resources responsibly, improve services, and build strong partnerships.
