Private Water System Permit Application
A permit application is required to be submitted prior to the construction, alteration, or sealing of any private water system. All permit applications must be complete and include a site plan
Additional Site Plan Form
As required in Ohio Administrative Code (OAC) 3701-28-03 (F), additional plans must be submitted to the Health Department as part of the permit application in certain situations. This form can be found here
