PORT is a referral-based program that connects individuals and families with resources, support, and a path forward after an overdose.

We receive a referral after an overdose or substance-related health event.

A trained team member attempts to contact the individual in a respectful and confidential manner.

We listen, answer questions, identify needs, and discuss available options.

We help connect individuals and families with appropriate community resources and support.

Our team can help connect you with a variety of supportive services, including:
✔ Treatment and recovery resources
✔ Support for family members
✔ Peer and community support
✔ Transportation assistance when available
✔ Harm reduction education and supplies
✔ Help accessing basic needs and social services
✔ Mental health resources
✔ Follow up support when appropriate
What to Expect
✔ You will be treated with dignity and respect
✔ The conversation is focused on your needs
✔ Participation is always voluntary

✔ Family members may receive support and education
